Intermediate Care

 Services which are provided to promote independence. They are given to those people who could face long hospital stays, long-term residential care and continuing NHS care. It involves assessment of each individual’s care plan. This takes the form of care designed to maximise independence and to enable patients and service users to return home. Intermediate care can last two to six weeks and involves support from a multi-disciplinary team.
